New Patient Information & Forms

Thank you for coming to our website to find out more about our practice and what we can do for you and for your family. We accept most insurance plans, and have the best hours for working people. Remember to bring along your newest insurance cards, and bring us a copy when you get your new cards each year.

 

We have three parts to our Practice: 1) Family Practice, which will provide 99% of your health care needs, including minor surgery, 2) Simeons Weight Loss Protocol Program, which has the highest success for weight loss in the State!, and 3)Microvas Therapy, which heals wounds and injuries by creating a unique bi-phasic energy field. 

 

We're open from 12 noon until 8p.m. Monday through Friday and from 9 a.m. until 5 p.m. on Saturdays. Please remember: while we give preference to seeing scheduled patients we always accept 'work-in or walk-in' visits. Below are some of the 'new patient' forms you'll need to fill out before being seen. You can print out and fill out the forms at home to save you time at registration.

Information About Our Practice

In order to make sure your visit to the practice goes smoothly, we've put together some simple but important information about how things work at the practice.

In order to register and treat you promptly, we request that our patients bring their current insurance information and photo ID with them for each visit. If you do not have your insurance information and photo ID with you, your appointment will be rescheduled.

We accept most types of health insurance. Please contact us for further details.
